Please use the form below to choose your option(s) for shirt(s).  The three choices are short sleeve t-shirts, long sleeve t-shirts, and long sleeve zip-up sweatshirts.  Sizes available are small, medium, large, extra large, 2X, 3X, & 4X.  Orders must be placed no later than November 16th.  We can take orders at the November 16th meeting, but NO orders will be accepted for this discounted rate after November 16th.
